In consideration of Maumee Tackle LLC allowing me to participate in this tournament, I acknowledge and agree as follows:
- I have read and understand the description of the Maumee Tackle Walleye Tournament as well the Official Rules of this tournament. I will follow all rules and I understand that a violation will result in immediate disqualification without any refund. I also understand that the tournament officials may reject my application for any reason and in that event will refund my entire entry fee.
- I acknowledge and agree that I understand the nature of competitive fishing, and related activities, and that I am qualified in good health, including being in proper physical and mental condition to participate in such activity. I further agree that I have no medical or physical condition that could interfere with my safety, or the safety of others around me, while participating in this activity, or else I am willing to assume and bear the costs of all risks that may result, indirectly or directly, by any such condition.
- I recognize and acknowledge that there is significant risk within competitive fishing, the risks of include, though not limited to, drowning, collisions in the water, and injuries from hooks and other fishing paraphernalia. I for myself and on behalf of my heirs, personal representatives, and next of kin, release and hold harmless Maumee Tackle LLC, officials, and employees. I knowingly and freely assume all such risks, both known and unknown, and assume full responsibility for my participation, including damage to myself, my property or to others, including other participants and their property, and agree that no legal action will be taken against Maumee Tackle LLC.
- I agree that I have adequate insurance to cover any injuries/damages that I may suffer or cause while participating in this activity. If I do not, I agree to bear the full costs myself.
- I hereby release, discharge, and agree to hold harmless, Maumee Tackle LLC, officials and employees from any and all liability, claims, demands, actions or rights of action which are related to, arise out of or are in any way connected with my participation in this tournament, including specifically, but not limited to, the negligent acts or omissions of Maumee Tackle LLC, and their partners, agents or employees and all other persons or entities associated with the tournament, for any injury, death, illness or disease, and other damage or loss to property suffered by myself or others. In signing this document, I acknowledge and agree that if anyone is hurt or property is damaged while I am engaged in this event, I will have no right to make claim or file a lawsuit against Maumee Tackle LLC, and their officials, employees and all other persons or entities associated with the tournament, even if they or any of them negligently caused the bodily injury or property damage.
- I give Maumee Tackle LLC permission to use my name and photograph for any promotional purposes, including social media post.
